[spa] La Nefropatía de los agricultores de Sri Lanka (SAN), es una enfermedad renal crónica
(ERC) que se caracteriza por no estar relacionada con ninguna de las causas típicas de la
ERC y estar ligada a la agricultura. Afecta desde hace más de 20 años a las
comunidades agrícolas de la North Central Province (NCP), y mayormente a hombres
de Sri Lanka. Presenta unas características histopatológicas de nefritis túbulointersticial,
esclerosis glomerular y atrofia tubular, que es congruente con la exposición
a nefrotóxicos. Se han hecho bastantes estudios durante años, que no han logrado
esclarecer la etiología, aunque si hay cierto consenso en que el origen es multifactorial.
Se han barajado varias hipótesis y se han estudiado algunos factores nefrotóxicos
presentes en el ambiente, como el Cd, y As, y los pesticidas, en concreto los herbicidas
en base a glifosato (GBHs), A partir de los resultados de los estudios epidemiológicos y
de otros estudios experimentales, se ha llegado a la conclusión de que puede existir una
relación sinérgica entre la exposición a MP, como el As, y a GBHs unidos a un agua
dura con presencia de cationes como Ca2+ y Mg2+, y que esta pueda estar suponiendo el
mayor riesgo para padecer CKDu en el área endémica de SL.
[eng] The Nephropathy of farmers of Sri Lanka (SAN), is a chronic kidney disease (CKD)
that is characterized by not being related to any of the typical causes of CKD and be
linked to agriculture. It has affected the agricultural communities of the North Central
Province (NCP) of Sri Lanka for more than 20 years, mainly men. It presents
histopathological characteristics of tubulo-interstitial nephritis, glomerular sclerosis and tubular atrophy, which is consistent with exposure to nephrotoxic agents. There have
been many studies for years, which have not managed to clarify the etiology, although
there is some consensus that the origin is multifactorial. Several hypotheses have been
considered and some nephrotoxic factors present in the environment have been studied,
such as Cd, and As, and pesticides, specifically glyphosate-based herbicides (GBHs),
based on the results of epidemiological studies and from other experimental studies, it
has been concluded that there may be a synergistic relationship between heavy metal
exposure, such as As, and GBHs linked to hard water with the presence of cations such
as Ca2 + and Mg2 +, and that this may be assuming the greatest risk for CKDu in the
endemic area of SL.
