La conspiración de silencio: manejo

Abstract

[spa] Objetivo: Definir el concepto de pacto o conspiración de silencio, conocer los motivos por los que los familiares o médicos suelen ocultar al paciente la verdad sobre el diagnóstico, conocer las razones por las que la CS va en contra de los principios éticos de una buena práctica clínica, cómo afecta al paciente y cómo podemos abordar esta situación. Material y métodos: Se ha realizado una revisión bibliográfica de artículos relacionados en las bases de datos BVS, PubMed/Medline, IBECS, CINAHL/EBSCOhost, CUIDEN y Google Académico. También se ha seleccionado información de libros y de páginas webs institucionales. Para realizar la búsqueda se han introducido los criterios de inclusión: “año de publicación en los últimos años” y “idioma castellano e inglés”. Resultados: Han sido incluidos 20 artículos, de los cuales 2 han sido tesis doctorales relacionadas con nuestro estudio y un artículo encontrado en una página web. En los artículos encontrados, hemos podido dar respuesta a nuestros objetivos específicos, hablando sobre las causas, consecuencias y estrategias relacionadas con la conspiración de silencio. Conclusiones: La conspiración de silencio es un hecho que ocurre muy a menudo en situaciones de enfermedades terminales y donde se observa un distanciamiento entre el paciente, la familia y los profesionales, al ocultarse información y crear desconfianza. Esta CS dificulta el afrontamiento ante la muerte del enfermo y está demostrado que al decir la verdad se mejoran las condiciones del final de la vida y se asegura una “muerte digna”.

[eng] Objective: Define the concept of pact or conspiracy of silence, know the reasons why relatives or doctors usually hide the truth about the diagnosis from the patient, know the reasons why the CS goes against the ethical principles of a good clinical practice, how it affects the patient and how we can approach this situation. Material and methods: A literature review of related articles was carried out in the VHL, PubMed / Medline, IBECS, CINAHL / EBSCOhost, CUIDEN and Google Scholar databases. Information about books and institutional websites has also been selected. To carry out the search, the inclusion criteria have been introduced: "year of publication in recent years" and "Spanish and English language". Results: Twenty articles have been included, of which 2 have been doctoral theses related to our study and one article found on a web page. In the articles found, we have been able to respond to our specific objectives, talking about the causes, consequences and strategies related to the conspiracy of silence. Conclusions: The conspiracy of silence is a fact that occurs very often in situations of terminal illnesses and where there is a distancing between the patient, the family and the professionals, by hiding information and creating distrust. This CS makes it difficult to cope with the death of the patient and it is demonstrated that by telling the truth the conditions of the end of life are improved and a "dignified death" is assured.
