Eficacia del tratamiento con plasma rico en plaquetas como coadyuvante al tratamiento quirúrgico y rehabilitador en deportistas jóvenes con rotura del ligamento cruzado anterior

[spa] Introducción: Dentro de las lesiones ligamentosas de rodilla, las cuales están frecuentemente asociadas a la práctica deportiva, la rotura del ligamento cruzado anterior (LCA) tiene una incidencia relativamente alta. Después de la reconstrucción del LCA, los pacientes están sometidos a un periodo largo de rehabilitación. En base a esta idea, se han investigado diferentes alternativas para reducir el tiempo de recuperación. El PRP se ha convertido en una herramienta prometedora para mejorar la curación después de la reconstrucción del LCA. Objetivo: Comparar la eficacia y el tiempo de recuperación del tratamiento con PRP en relación al tratamiento quirúrgico y rehabilitador habitual en deportistas con rotura del Ligamento Cruzado Anterior. Métodos: Búsqueda bibliográfica en PUBMED, COCHRANE, SCIELO, PEDRO y SPORTDISCUS (EBSCOHOST). Los criterios de inclusión fueron: Humanos, preferiblemente deportistas, con rotura del LCA como lesión ligamentosa de rodilla, intervenidos quirúrgicamente con una reconstrucción completa del LCA, uso del PRP como tratamiento suplementario a la cirugía intra o post intervención, incluyendo todo tipo de material de PRP en los diferentes estados (gel, líquido, etc.). Resultados: Se incluyeron 12 ensayos para ser revisados. Tras el análisis, se observaron posibles efectos significativos para evaluar la presunta eficacia del PRP en la reconstrucción del LCA, aunque con una gran variabilidad entre los resultados de los diferentes estudios. Conclusión: Los resultados de esta revisión muestran que no existe demasiada evidencia tanto para asegurar o descartar actualmente la utilidad del PRP como coadyuvante al tratamiento quirúrgico y rehabilitador de las reconstrucciones del LCA.

[eng] Introduction: Within the ligamentous injuries of the knee, which are frequently associated with sports, the rupture of the anterior cruciate ligament (ACL) has a relatively high incidence. After ACL reconstruction, patients undergo a long period of rehabilitation. Based on this idea, different alternatives have been investigated to reduce recovery time. PRP has become a promising tool to improve healing after ACL reconstruction. Objective: To compare the efficacy and recovery time of PRP treatment in relation to the usual surgical and rehabilitative treatment in athletes with ruptured Anterior Cruciate Ligament. Methods: Bibliographic search in PUBMED, COCHRANE, SCIELO, PEDRO and SPORTDISCUS (EBSCOHOST). The inclusion criteria were: Humans, preferably athletes, with ACL rupture as a knee ligament injury, who underwent surgery with a complete ACL reconstruction, use of PRP as a supplementary treatment to intra or post-intervention surgery, including all types of surgical material. PRP in the different states (gel, liquid, etc.). Results: Twelve trials were included for review. After the analysis, possible significant effects were observed to assess the presumed efficacy of PRP in ACL reconstruction, although with great variability between the results of the different studies. Conclusion: The results of this review show that there is not too much evidence to currently assure or rule out the usefulness of PRP as an adjunct to the surgical and rehabilitative treatment of ACL reconstructions.
