Impacto de un programa de intervención nutricional en ancianos hospitalizados con disfagia orofaríngea

[spa] Introducción y objetivos: La disfagia orofaríngea en pacientes ancianos está asociada a malnutrición, declive funcional, mayor estancia hospitalaria y mortalidad, por lo que su presencia tiene implicaciones pronósticas. Los programas de intervención para pacientes con este problema que combinan la intervención tanto nutricional como educativa consiguen evitar la desnutrición y/o mejorar el estado nutricional y evitar el deterioro funcional de estos pacientes cuando existe una adecuado cumplimiento de las recomendaciones higiénicodietéticas. El objetivo principal de este estudio es intervenir sobre la evolución natural del estado nutricional de estos pacientes mediante un programa bidimensional. Material y métodos: Estudio cuasi-experimental antes y después de la implantación de un programa de intervención nutricional. Se incluirán pacientes mayores de 65 años ingresados en el servicio de medicina interna del Hospital Morales Meseguer en los cuales se haya documentado disfagia mediante el método de evaluación clínica volumen-viscosidad (MECV-V). Se analizará al ingreso el estado nutricional mediante el Mini Nutritional Assesment, el estado funcional mediante el índice de Barthel y se evaluarán los conocimientos del cuidador sobre las medidas higiénico-dietéticas que se consideran indicadores de calidad en la atención a la disfagia. Se analizará el estado nutricional y funcional de estos pacientes así como el cumplimiento por parte del cuidador de las citadas medidas a los 6 meses de la implementación del programa. Aplicabilidad y utilidad de los resultados: Demostrar la importancia de la intervención nutricional sobre la disfagia orofaríngea y de la formación del cuidador en las medidas higiénico dietéticas con el fin de incentivar la sistematización de su cribado y tratamiento en la práctica clínica para mejorar los estándares de asistencia sanitaria.

[eng] Introduction and objectives: Oropharyngeal dysphagia in elderly patients is associated with malnutrition, functional decline, longer hospital stay and mortality, so its presence has prognostic implications. Intervention programs for patients with this problem that combine both nutritional and educational intervention manage to avoid malnutrition and / or improve nutritional status and avoid functional deterioration in these patients when there is adequate compliance with the hygienic-dietary recommendations. Nutritional status will be analyzed upon admission using the Mini Nutritional Assessment, functional status using the Barthel index, and the caregiver's knowledge of the hygienic-dietary measures that are considered quality indicators in dysphagia care will be evaluated. The nutritional and functional status of these patients will be analyzed, as well as compliance by the caregiver with the 6 months after the implementation of the program. Applicability and usefulness of the results: Demonstrate the importance of nutritional intervention on oropharyngeal dysphagia and the training of the caregiver in dietary hygiene measures in order to encourage the systematization of its screening and treatment in clinical practice to improve the standards of healthcare.
