Variables físiques, afectives i cognitives en adults amb fibromiàlgia: estudi de casos i controls

[cat] La fibromiàlgia (FM) és una síndrome que es caracteritza per dolor musculoesquelètic generalitzat i de més de tres mesos de durada, amb una major prevalença en dones entre els 60 i 69 anys, que produeix la incapacitació a totes les àrees de desenvolupament personal (laboral, social i familiar). Els pacients amb FM informen de queixes cognitives, com problemes de concentració, oblits i falta d’atenció. Així i tot, els resultats dels estudis que s’han realitzat per analitzar objectivament el rendiment cognitiu són discordants. L'objectiu general d'aquest treball va ser explorar les afectacions a nivell físic, afectiu i cognitiu, i les seves relacions, en un grup de pacients amb FM en comparació amb un grup control de persones sanes (sense FM), tenint en compte el context actual de pandèmia per la COVID-19. Els resultats apunten que els pacients amb FM presenten deteriorament i/o problemes físics, major simptomatologia depressiva i ansiosa que correlaciona amb els símptomes generals de la FM (intensitat i interferència del dolor, funcionament físic, fatiga, alteració del son, capacitat funcional de participació social), major simptomatologia depressiva, ansiosa i d’estrès posttraumàtic degut a la irrupció de la COVID-19, i pitjor funcionament cognitiu tant auto-percebut (dificultats de concentració i de memòria) com alhora de realitzar tasques de vigilància executiva (més errors) i de vigilància atencional (més lents), en comparació al grup control. En conclusió, aquest estudi posa en evidència els problemes físics i afectius, a més de queixes i certes dificultats cognitives, principalment de vigilància/control executiu, als quals fan front les persones amb FM que, en conseqüència, produeixen un empitjorament de la seva qualitat de vida.

[eng] Fibromyalgia (FM) is a syndrome characterized by generalized musculoskeletal pain lasting more than three months, with a higher prevalence in women between 60 and 69 years old, which produces disability in all areas of personal development (work, social and family). FM patients report cognitive complaints, such as concentration problems, forgetfulness, and inattention. However, the results from studies conducted to objectively analyze cognitive performance are discordant. The general aim of this work was to explore the physical, affective, and cognitive impairments, and their relationships, in a group of patients with FM compared to a control group of healthy people (without FM), considering the current context of the COVID-19 pandemic. The results indicate that patients with FM present deterioration and/or physical problems, greater depressive and anxious symptoms that correlate with the general symptoms of FM (intensity and interference of pain, physical functioning, fatigue, sleep disturbance, functional capacity for participation social), greater depressive, anxious and posttraumatic stress symptoms due to the emergence of COVID-19, and worse cognitive functioning both self-perceived (concentration and memory difficulties) as well as performing tasks of executive vigilance (more errors) and attentional vigilance (slower), compared to the control group. In conclusion, this study highlights the physical and affective problems, as well as complaints and certain cognitive difficulties, mainly of vigilance/executive control, which people with FM must face, and consequently, producing a worsening of their quality of life.
